How to Submit a claim

1. Download a claim form. Check out your Customer Portal to download your personalized claim formor click on one of the links below.
2. Visit your veterinarian. After you pay your bill, ask your veterinarian or an authorized clinic employee to fill out sections 2 and 3 of the claim form. You fill out sections 1 and 4.
3. Submit your claim. Send us your completed claim form.

Take a photo of your claim form and invoice and submit it through your customer portal
